This portal is for patients completing the McCairn Edogawa DFPA protocol. Use it to record how your Long COVID symptoms and daily life change over time, and to give your clinical team consistent information at each follow-up.

Long COVID symptoms can fluctuate. If activity causes a delayed flare, reduce the load and discuss pacing with your clinical team.
Rate the same symptom and daily-life questions each time. Consistency makes your pre/post record more useful.

Do not use a questionnaire to ask for urgent help. For severe or sudden symptoms, contact local emergency services. For non-urgent concerns, contact McCairn Edogawa clinical team.
WHAT YOUR CHECK-IN DOES
Record fatigue, activity-related worsening, cognition, breathing, circulation, sleep, pain, and other changes.
Show how symptoms affect personal care, mobility, work, thinking, and social participation.
Record the DFPA session and SGF information connected with that check-in.
The score supports consistent tracking. It does not diagnose illness or prove that treatment caused a change.
